What this notice covers
This privacy notice applies to mdpsychassociates.com, the marketing website for Maryland Psychiatric Associates. It is not a HIPAA Notice of Privacy Practices — that document covers the clinical practice and is provided separately when you become a patient.
What we collect
If you fill out the contact form on this site, we collect the information you provide:
- First and last name
- Email address
- Phone number
- Visit preference (in-person, telehealth, or no preference)
- A brief sentence about what brings you to us
We ask that you do not include detailed medical history, diagnoses, or sensitive clinical information in this form. After we connect, you'll receive a secure intake link through our HIPAA-compliant electronic health record system, where it is appropriate to share that information.
Our hosting provider (Netlify) automatically logs basic technical information about visitors — IP address, browser type, pages viewed — for security and operational purposes. We do not use marketing analytics or third-party tracking cookies on this site.
How we use it
We use the information you submit to:
- Respond to your inquiry and arrange a brief consultation
- Verify, with your consent, your insurance benefits prior to any visit
- Maintain a record of your inquiry as required by professional standards
How we share it
We do not sell, rent, or trade your information. Form submissions are transmitted from Netlify directly to a HIPAA-compliant email address controlled by Maryland Psychiatric Associates. The third parties involved in operating this site are:
- Netlify — website hosting and form processing
- Google Fonts — typography (loads font files from Google's servers)
Your rights
You may request that we delete the contents of any inquiry you submitted at any time. Email zach@mdpsychassociates.com with the subject line "delete my inquiry" and the email address you used.
Children's privacy
This website is not directed at children under 13. While Zach treats children and adolescents in his practice, the website's contact form is intended for use by parents, guardians, or adult patients. Do not have your child fill out the form on their own.
Security
We use HTTPS encryption for all traffic on this site. Form submissions are transmitted over encrypted channels. The website itself does not store your submission — it is delivered to a secure email address and any further communication takes place through HIPAA-compliant channels.
Changes to this notice
We may update this notice from time to time. The "effective date" at the top will reflect the most recent change. Substantial changes will be flagged on this page.
